FormsSave time at your first appointment! Download the appropriate set of forms, fill them out, and bring them with you to your first appointment. Your counselor is looking forward to seeing you at HOPE Network. Either arrive 15 minutes early and fill out the intake forms in our reception area, or download them from this site and bring them with you. At your first appointment, your counselor will review the counseling relationship with you and discuss items such as confidentiality, fees, and your rights and your responsibilities as a client of HOPE Network. If you want to complete and print out your paperwork, please do the following: 1. Open the appropriate set of forms and type your information on the form. 2. Print your completed forms on your printer. 3. Bring the forms to your first appointment.
The forms require the free program Adobe Reader, which is already installed on most computers. You can download a free copy of Adobe Reader at www.adobe.com.
